If you move forward with a clinical study, you will receive study-specific informed consent materials and, when applicable, a HIPAA Authorization or other privacy notice describing how your health information may be used and disclosed as part of the research.

• Website and form information: When you use an online form, certain technical information, such as your IP address, may be collected for website functionality, security, and spam prevention.

We may use the information you provide to:

• Respond to your inquiry and contact you about research opportunities.
• Determine whether you may meet preliminary eligibility criteria for a study.
• Schedule or conduct additional screening when appropriate.
• Document recruitment and screening activities as required by applicable study, regulatory, sponsor, or Institutional Review Board (IRB) requirements.

3. How We Collect Information

We may collect information:

• Through forms you complete on our website.
• During phone calls, email exchanges, messages, or other communications with our research team.
• Through study-specific screening questionnaires or recruitment tools.
• From a healthcare provider or other referral source when the referral and disclosure of information are permitted or authorized.

4. How We Protect Your Information

SRI uses reasonable administrative, technical, and physical safeguards designed to protect personal and health information collected during recruitment and pre-screening.

These safeguards may include:

• Restricting access to authorized personnel who need the information for recruitment, screening, research operations, or regulatory responsibilities.

• Within SRI: Information may be accessed by authorized SRI staff involved in recruitment, screening, study operations, regulatory oversight, or other responsibilities related to the research.
• Study-related organizations: If you move forward with a study, information may be shared with the study sponsor, contract research organization, IRB, laboratories, research collaborators, or other study-related organizations when appropriate and as described in the study-specific consent and authorization materials.
• Legal or regulatory requirements: Information may be disclosed when required or permitted by applicable law, regulation, or research oversight requirements.
• No sale of recruitment information: SRI does not sell personal information collected for research recruitment or pre-screening.

6. Your Choices and Requests

Before enrollment in a clinical study, you may:

• Ask questions about the information you have provided to SRI.
• Request that information you provided be corrected or updated.
• Tell us that you no longer wish to be contacted about a study or future research opportunities.
• Request deletion of pre-screening information when deletion is permitted and the information is not required to be retained for research, regulatory, legal, or recordkeeping purposes.

Once you enroll in a clinical study, your rights regarding study records and health information will be described in the study-specific informed consent and privacy authorization materials.

7. How Long We Keep Information

Pre-screening data is retained only as long as reasonably necessary for recruitment, research recordkeeping, and applicable legal or regulatory requirements. If you do not enroll in a study, study-specific screening records may be retained for the period required by the study protocol, sponsor, Institutional Review Board (IRB), or applicable law.
